What is the best/safest way to remove mildew in the carpet??

Scrubbing Bubbles worked great on my heavily mildewed vinyl. It might be worth a shot on your carpet... maybe try a small area.
Spray a small area, then scrub with a stiff brush, then rinse really well. Then I'd shampoo the carpet.
Just make sure you rinse well. Residue from cleaning products will attract dirt quickly.
